I’M LOGAN IT FOUNDATION

1989 Pursuit 2650 Powerboat Raffle

Official Rules

I’M LOGAN IT Foundation, a fund of the Pinellas Community Foundation, Inc., a Section 501(c)(3) tax-exempt organization (the “Foundation”), is conducting a single-prize chance drawing (“Raffle”) of a completely remodeled 1989 Pursuit 2650 Powerboat (the “Prize”) pursuant to Florida Statutes Section 849.0935 to raise funds to benefit the I’M LOGAN IT Foundation.  The Prize has been graciously donated by TACO Metals, Inc. (“Donor”).  The winning ticket will be drawn at random on or about Nov. 5, 2017 at 1:00 p.m. EST during the Fort Lauderdale International Boat Show, from all eligible raffle tickets received. In addition, the winner will be notified by phone. Ticket holders do not need to be present to win.  Detailed Official Rules follow.

ELIGIBILITY.  The Foundation has the right to request photo identification from any person desiring to enter the Raffle. The Raffle is void where prohibited by law, rule, or regulation. All federal, state, and local laws and regulations apply. Employees of the Foundation, the Donor, and members of their immediate family and residents of their household, are not eligible to enter. “Immediate family” includes only an employee’s spouse, parents, grandparents, children, and employee’s siblings living in the same household. A request for and receipt of a Ticket constitutes the entrant’s full and unconditional agreement to and acceptance of these rules, terms and conditions, including all entry requirements.

No purchase or contribution is necessary to enter or to win the Raffle. To enter, an eligible entrant must request and receive from a designated representative of the Foundation an official raffle ticket (“Ticket”) for the Raffle.  Each Ticket will bear a unique identification number (“Ticket Number”) and those numbers shall be used to determine the winner of the Raffle. There are suggested minimum cash contributions to the Foundation in connection with the receipt of Tickets (with the total amount of all such contributions made in connection with the receipt of Tickets, as determined by the Foundation, in its sole discretion, being hereinafter referred to as the “Raffle Proceeds”). Tickets may be obtained upon request in-person or by mail, or upon automatic entry with a donation at www.IMLOGANIT.com.  There is a maximum of one (1) Ticket per eligible person if no donation is made by such person.  To request a Ticket by mail, either with or without a donation, submit a written request, which must include first and last name, address, e-mail address and phone number, along with a self-addressed stamped envelope to:  The I’M LOGANIT Foundation c/o TACO Metals, Attn:  Project Boat Raffle Fundraiser, 50 N.E. 179th Street, Miami, FL  33162.

DRAWING AND NOTIFICATION OF PRIZE WINNER.  The potential winner of the Raffle will be selected by random drawing from all eligible entries actually received by the Foundation on or before November 4, 2017, with the drawing to be held at the Fort Lauderdale International Boat Show on November 5, 2017 at 1 p.m. EST. The holder (“Raffle Winner”) of the Ticket bearing the winning number will be contacted within five (5) business days following the drawing to present the winning Ticket to the Foundation within ten (10) business days following the drawing.  Entrants need not be present to win. The winning number for the Raffle shall be posted (“Website Posting”) at www.IMLOGANIT.com on the day immediately following the drawing and shall remain posted throughout the Claims Period (as that term is defined hereinafter) or can be obtained by calling 305-652-8566 during normal business hours. In order to be eligible to claim the Prize, the Raffle Winner must deliver the winning Ticket to a Foundation representative within the time period (the “Claims Period”) that ends at 5:00pm EST on the day that is ten (10) calendar days after the Raffle was held (provided, however, that if such tenth day falls on a Saturday or a Sunday, then the Claims Period shall be extended such that the Raffle Winner shall have until the Monday immediately following such tenth day to deliver the winning Ticket in order to be eligible to claim the Prize).  Any transportation required for the Raffle Winner to make delivery of the winning Ticket shall be the Raffle Winner’s sole responsibility and will not be furnished or paid for by the Foundation.  Upon presentation of the winning Ticket to the Foundation during the Claims Period and verification by the Foundation that it bears the winning number and is the authentic winning Ticket (with the date upon which the winning Ticket is presented to the Foundation being hereinafter referred to as the “Delivery Date”), the Ticket will be retained by the Foundation, the Raffle Winner shall provide his or her photo identification and one other form of identification as noted below, mailing address (“Mailing Address”) and telephone number, and the Foundation shall provide the Raffle Winner with written verification that he or she has won the Raffle. The Raffle Winner will be required to execute and deliver to the Foundation such documents as reasonably requested by the Foundation at the time the Raffle Winner delivers the winning Ticket to the Foundation.

The Foundation will award the Prize only upon verification of the Raffle Winner and the winning Ticket in the Foundation’s sole discretion. Decisions by the Foundation as to Raffle Winner eligibility and acceptance or disqualification will be final and binding. If the potential winner has not complied with these Official Rules, including the delivery of the winning Ticket, the provision of his or her photo identification and another acceptable form of identification, Mailing Address and telephone number, and the execution of the Affidavit of Eligibility and Liability/Publicity Release and W-9 tax form during the Claims Period, the Prize will be forfeited and shall be retained by the Foundation.  In the event the potential winner does not accept the Prize, is disqualified for any reason, or fails to comply with these Official Rules, the Prize will be forfeited and shall be retained by the Foundation. Upon Prize forfeiture, no compensation will be given to the potential winner.  Failure of any potential winner to sign any requested document or failure of any potential winner to comply with any terms in these Official Rules may disqualify the winner in the Foundation’s sole discretion and the potential winner may forfeit the Prize.  If no one presents the winning Ticket to the Foundation prior to the end of the Claims Period in accordance with the procedures set forth above, the Prize will be forfeited and shall be retained by the Foundation. The Prize will not be awarded to any other person in this Raffle.  The Foundation will not be required to attempt to notify or locate the holder of the winning Ticket in any manner other than by making the Website Posting and mailing notice to the address on the winning Ticket. The Foundation also will not be responsible if the holder of the Winning Ticket does not receive the notification, check the Website Posting, call the Foundation Telephone Number, is otherwise unaware that he or she has the right to collect the Prize, or does not present the winning Ticket within the required time as set forth in these Official Rules. If the holder of the winning Ticket attempts to deliver the winning Ticket to the Foundation and collect the Prize at a later time or date, he or she will be notified that the Prize has been forfeited.

PRIZE RESTRICTIONS AND ODDS: The Prize is not transferable or assignable and cannot be substituted for another prize. Any and all taxes, fees, and other costs including, but not limited to, federal and, if applicable, state and local income and withholding taxes that may arise and be due or asserted on the value of the Prize, are the sole responsibility of the Raffle Winner. Applicable federal tax law requires income tax withholding of 25% (at time of printing) of the fair market value of the Prize, which must be paid prior to delivery. Neither Donor nor the Foundation shall be responsible or liable for any actions or for any events that occur during or as a result of the Raffle Winner’s acceptance and use of the Prize.  The Raffle Winner will contact Donor for information on the completion of transaction and vehicle delivery.  The Raffle Winner shall be solely responsible for all expenses and arrangements for pick-up and delivery of the Prize from Miami, Florida.  No substitutions for the Prize will be made. For additional information, please contact the Foundation at:  I’M LOGANIT Foundation c/o Pinellas Community Foundation, 5200 E. PUBLICITY RELEASE: The Raffle Winner agrees that acceptance of the Prize grants to the Foundation, the Donor, and each of their respective related entities, the right to print, publish, broadcast, and use world-wide and in perpetuity, in any media now known or later developed, at any time or times, the winner’s name, portrait, picture, voice, likeness, and biographical information for any purpose, including, but not limited to, in advertisements, publicity, and other communications without additional compensation, except where prohibited by law.

A note from the Juvenile Diabetes Research Foundation, Tampa Bay Chapter

It is because of the generosity of organizations like the I’M LOGAN IT Foundation that JDRF has the ability to make living with type 1 diabetes (T1D) safer and healthier, until there is a cure. The JDRF Tampa Bay Chapter truly appreciates the support of the I’M LOGAN IT Foundation and its annual donations to our organization totaling $65,000 over the past five years. Please know your support impacts the lives of the millions of children, adults and families challenged by T1D every single day.

Every detail of our organization is guided y a simple goal: to lessen the tremendous daily burden of this disease, until it is no longer a threat. That’s why we are busy putting your dollars to work funding research for life-changing advances like glucose-responsive insulin, an artificial pancreas or beta cell replacement therapy, while relentlessly pursuing our vision of creating a world without T1D.

We can assure you that we will stop at nothing until Type One becomes Type None. On behalf of the JDRF Tampa Bay Chapter and millions of people living with T1D, thank you for your continued support.

Sincerely,

Carolyn Boos

Palm Harbor Hurricanes win 5th annual Logan Kushner Memorial Wrestling Tournament

More than a dozen high school teams from across the state participated in the Logan Kushner Memorial Tournament this past February.

The Palm Harbor University High Hurricanes won for the fourth year in a row. The meet is a round-robin format where wrestlers in a certain weight class wrestle every other competitor in the class during the two-day meet.

Palm Harbor Hurricane Jake Kampman went 6-0 to win his 152-pound weight class. “I won this tournament in Logan’s honor,” said Kampman, who remembers Logan wrestling his other brother, Josh, and carpooling with them.
